Overview
Kidney stones are one of the most common urological conditions. Up to 50% of patients who form a kidney stone will experience another episode within five to ten years. With each recurrence, the risk of forming yet another stone increases further, creating a frustrating and painful cycle.
For patients who have had two or more stones, or who formed their first stone at a young age, a thorough metabolic evaluation is essential as it may reveal risk factors which can be addressed to reduce future stone risk.
Our goal is to tailor investigations to the individual patient and to break the recurrence cycle. Through appropriate surgical strategy, comprehensive metabolic testing, stone analysis, and personalised dietary and medical treatments.
By moving kidney stone management from purely surgical stone removal, to looking at the whole individual and their needs, we aim to reduce the risk of future stone episodes and protect long-term kidney health.

A comprehensive metabolic workup is the cornerstone of preventing recurrent stones. This involves three key investigations that together give us a detailed picture of why stones are forming.
24-Hour Urine Collection
The single most informative test. We measure:
- ›Total volume — ideally >2.5L/day
- ›Calcium — elevated in hypercalciuria
- ›Oxalate — high levels promote calcium oxalate stones
- ›Citrate — low citrate is a key risk factor
- ›Uric acid — elevated in uric acid stone formers
- ›Sodium — a marker of dietary salt intake
- ›pH — crucial for determining stone type risk
Blood Tests
Essential blood markers include:
- ›Serum calcium — to screen for hyperparathyroidism
- ›Parathyroid hormone (PTH) — if calcium is elevated
- ›Uric acid — elevated in gout and uric acid stones
- ›Renal function (eGFR, creatinine) — assessing kidney health
Stone Analysis
Every stone should be sent for laboratory analysis. This is essential because:

Prevention Strategies by Stone Type
The most effective prevention strategy depends on the type of stone you form. Here we outline the evidence-based approaches for each major stone type.
Calcium Oxalate Stones
The most common stone type — approximately 70-80% of all stones
Dietary Measures
- Increase fluid intake to >2.5L/day
- Maintain moderate calcium intake — do NOT restrict calcium (this actually increases stone risk)
- Reduce oxalate-rich foods (spinach, rhubarb, nuts, chocolate, beetroot)
- Reduce salt intake to <6g/day
Medical Therapies
- Potassium citrate — raises urinary citrate and pH
- Thiazide diuretics — reduce urinary calcium excretion in hypercalciuria
Uric Acid Stones
Account for 5-10% of stones — often associated with gout, obesity, and type 2 diabetes
Key Strategies
- Alkalinise urine — target pH 6.5-7.0 with potassium citrate (uric acid stones dissolve in alkaline urine)
- Allopurinol — reduces uric acid production if levels remain high
- Reduce purine-rich foods (red meat, organ meats, shellfish)
- High fluid intake — aim for >2.5L/day
Good news: Uric acid stones are the only stone type that can potentially be dissolved with medical therapy alone, by alkalinising the urine. This can avoid the need for surgery altogether.
Struvite (Infection) Stones
Caused by urease-producing bacteria — can grow rapidly into staghorn calculi
- Treat and prevent urinary tract infections — the underlying cause must be addressed
- Complete stone clearance — residual fragments harbour bacteria and lead to rapid recurrence
- Long-term low-dose antibiotic prophylaxis may be considered in selected patients
Cystine Stones
Rare inherited condition (cystinuria) — typically presents in childhood or young adulthood
- Massive hydration — 3L+ per day, including waking at night to drink, to keep urine dilute at all times
- Urine alkalinisation — target pH >7.0 with potassium citrate
- Tiopronin or D-penicillamine — cystine-binding drugs that reduce cystine concentration in the urine
- Reduce salt and animal protein intake
Lifestyle & Dietary Advice
Regardless of stone type, certain universal lifestyle measures significantly reduce the risk of recurrence. These are simple changes that can make a big difference.
Hydration
Aim for 2.5-3L of fluid per day. Your urine should be consistently pale straw coloured. Spread intake throughout the day and before bed.
Reduce Salt
Keep sodium intake below 6g/day (about one teaspoon). High salt directly increases calcium excretion in the urine, promoting stone formation.
Balanced Calcium
Maintain a normal calcium intake through food (not supplements). Restricting calcium paradoxically increases oxalate absorption and stone risk.
Limit Animal Protein
Excessive red meat, poultry and fish increase uric acid, lower citrate, and acidify the urine. Aim for moderate portions.
Healthy Weight
Obesity is a significant risk factor for both uric acid and calcium oxalate stones. Maintain a healthy BMI through diet and regular exercise.
Lemon Juice
Freshly squeezed lemon juice is a natural source of citrate, a potent stone inhibitor. Add it to your daily water intake for an easy boost.
Long-Term Monitoring
Prevention is a long-term commitment. Regular follow-up allows us to detect new stones early, adjust treatment, and ensure that preventive measures are working effectively.
Regular Imaging
Periodic ultrasound or low-dose CT scanning to monitor for new stone formation or growth of existing residual fragments. The frequency depends on your individual risk profile — typically annually or every 18 months.
Annual Metabolic Bloods
Yearly blood tests to check calcium, uric acid, and renal function. Any changes may indicate a new or evolving metabolic abnormality that needs to be addressed.
Compliance Review
We review your dietary and fluid intake, medication adherence, and repeat 24-hour urine collections if needed to confirm that prevention measures are having the desired effect on urine chemistry.
Medication Adjustment
Medications such as thiazides, potassium citrate, or allopurinol may need dose adjustments over time based on urine chemistry results, side effects, or changes in your overall health.
